Such as holding_with_v, holding_with_w, holding_with_\n/, holding_with_\m/, etc.
I would prefer a catch-all tag like holding_with_hand_gesture or holding_with_gesture first because I'm unsure about how often this type of holding comes up, but its probably worth a new tag.

You are right, if a user wants to search for characters holding a cup (or some object) with their pinky out, they can search holding cup + pinky out or use a more broad search like holding + pinky out and most posts will show holding with the same hand that they are raising their pinky. So adding holding with gesture is not too helpful for pinky out.
In my opinion, holding with gesture should be used for gestures that typically are not used to hold objects. Pinky out is a gesture, but it is not as rare to hold objects with that gesture than it is with middle finger, v, \m/ and thumbs up, which are very stylish ways of holding an object.
You might find different opinions in Discord tho

Tags can have a plus in them so just use 4RIN+.
