Testicle size has always been a pet peeve tag of mine because it doesn't actually get tagged all the often it seems. The lackluster wikis kinda leave it up to the judgement of the tagger. I agree it'd be nice if we had some kind of standard like breasts and penis, but I admit I can't think of a good scale. Like at what point do large_testicles (post #4647139) become huge_testicles (post #5424972) . Sometimes when tagging I think "If this character put on pants could you still tell they had massive nuts?" Yes? Then they have huge_testicles not just large ones." Gigantic_testicles is to me the only easy one to tag.
