That miiiight be because she was getting off on her own sadism and could barely contain herself. Keep in mind the artist behind this, happy endings aren't his usual thing so keep on guard.
The short answer: This is the climax of the story... and she just climaxed... hard.
The long answer: She is a sadere (sadism-version of tsundere) according to the artist, and so, like a tsundere, she shows her sadistic nature the more that she is incapable of dealing with her true feelings. As she just got confessed to, in spite of being totally unprepared for it (see the stuttering she does over the word "suki"/"like") and trying to push the guy away with her verbal abuse, she's reaching a stress level near the melt-down point, and is hence converting that all into her sadism fetish - hence the heavy breathing, twitching, tears, and the gratefully relieved face she has in the next frame.
