In Elona, Spiral King is a high level endgame monster.

A strong enough player can capture one as a pet, which is what this picture depicts.

Or rather, she'll be the one captured. :3

Given the heart speech bubble, it's possible this is an already-captured spiral king who's just being "friendly" with their owner.

Incidentally, Elona is a game with absolutely no restrictions on relationships. If you can keep it as a "pet" (which is what they also call humanoid allies and slaves), you can marry it and have "genes" (children) with it - regardless of gender or species or completely incompatible anatomy. A "male" golem (whatever that means) can marry a boy android and have children.
