Is there another figure atop the snakes head beside Lady Yasaka? It looks like someone in a hood.

Most likely that's Sanae's skirt.

Sanae is most likely, though there is a miniscule chance it might be Ichirin. You can see it as either a skirt over legs, or a hood over a face. (Maybe this could be like one of those classic "optical illusion" pictures...)
