    Flat explanation of image follows. It's a common trope in Japanese art (can't say for western since the style generally doesn't interest me) for the girl to want to stifle her noises; Kaga couldn't manage and thus stuffed her face in the pillow. Partner wasn't having that, so yoink.

    And, it's adorable.

    It's probably a sort of cultural thing shaped by the fact that living space is precious in Japan, and you wouldn't want the neighbors to hear your sex partner screams through the thin walls.
