If anyone needs context, that is a mimikaki. Unlike a cotton swab, they are actually effective at ear cleaning, although they have the potential to be injurious if the user isn't careful (they aren't known for it, but since these will be going deep inside an ear, it goes without saying...)
Traditionally, these are used by Japanese mothers on their children, and it's considered a very close bonding exercise because of that. For someone to accept ear cleaning from an unrelated person requires a great deal of trust and closeness.
