Depending on the size relative to the torso, and if the waist is clearly incapable of supporting that load, they can. Also, there's a limit to "gigantic" for Danbooru of triple head size.

Neither really applies, here, I think it's the man that is causing the flag.

Breasts aren't always perfectly equal in size, in fact they're almost never perfectly equal in size in real life. If there's a huge difference in size, by all means point it out; but slight differences in size are more realistic than none at all.

I don't care that you're not the flagger (I don't even like this particular post), I'm telling you that slightly differently sized breasts are neither unrealistic nor inherently badly proportioned.

Eh,the tag isn't even there. But maybe should be added.....
But it got apparantly got approved although of giant hands of the girl, very big head of the old man and a too long right arm (it's longer as the left one, too). The breasts are they really the smallest problem here. Still a problem, though.
