I think the ultra beast would be more confused then surprised when they see a penis because they live in a world where there are no humans, so human genitals would be unknown. (But then again this is fan art so why am I questioning it?)

Not only that, but none of the Ultra Beasts have genders, so for all Pheromosa knows, that's just a strange third leg.

For the most part, legendary pokemon only do not have genders for the sake of game breeding mechanics. Latios, Latias, Lugia, Mewtwo and many others have been shown in the anime/manga to be male or female despite the game mechanic. So Pheromosa could be female (and likely is, do to the female characteristic) But like kyurem said, it is fan art so whatevs.
