There is, in fact, a clear point of reference, because her feet are on said penis, and it's at least as big as just one of them. It's thicker than her ankle. She could probably sink her pinky in his urethra to the third knuckle without major discomfort to either party. No, sir, that is an absurdly large penis.

According to her info on her wiki page, she's only around 4'11, and judging from her height combined with her small stature, one could come to the conclusion that her foot would be relatively small like the rest of her, which in turn gives a relative idea as to her foot size, also considering the fact she weighs approximately 75 or 80 pounds. All of which makes me think Rukia is just a very small woman and his penis is of "average" size, about 5 or 6 inches in length. A little food for thought.

Let's assume, just to have a point of reference, that her big toes are ¾ inch wide (mine are about 1¼ inch wide and I wear a men's US 9½ regular shoe. That makes hers generously small). The penis she's stroking with said toes is visibly 5 to 6 times as wide — 3¾ to 4½ inches. Average is closer to 1½ inches wide, maybe 1⅝.
Let's make her big toes smaller still, just ½ inch instead of ¾. This penis still comes out to at least 2½ inches wide, about the same as my wrist at it widest. I don't think you can make this an average-sized penis without making her something like 3 feet tall or less.
