Grammatically it parses as a single word with a long prenominal predicate (i.e. a sentence converted into a descriptor for a noun) attached to it. Doesn't sound unnatural in Japanese, but needs some rephrasing when translated as trying to reproduce the grammatical structure in English would end up sounding awkward.
I reworded it as follows to hopefully make it more easily understood:
••••「正体がわたしだ」 ••とも知らないのに ••慌てふためく アイツの顔
With some of the more archaic/poetic words switched out for more modern ones, plus using bullet points to indicate the level of nesting.
