I don't know about the situation in Gensokyo, :) but it's very rare to find left-hand drive cars in Japan.

I used a Lamborghini Gallardo as reference.

Artist possibly found an interior reference picture on the net, does not drive enough to notice and did not think further about it while putting a relatively humongous Alice in its driver seat.

Eh,you'd be surprised. Last year when I spent a month in Tokyo there looked to be a good bit of imports (german mostly) that were LHD. I was also surprised to see a few classic american cars out and about.
