If it were fat, gravity would make it squish more out to the sides and she wouldn't have such a deep valley where her thigh meets the hip. If it were her quadriceps — not likely, since her knee is in flexion — one would expect to see more muscle definition along the length of her thigh, not just in one spot. Maybe it's supposed to be a tumor?
Or maybe the artist thinks people are made out of modeling clay and can be shaped like whatever with zero consideration for the structures underneath.
