For reference, later on, there are numerous "Sakuya Pads" jokes, and Patchouli is portrayed as something like a grade-schooler or kindergardener in this series (the same relative apparent age as Remilia), along with the other "masters" in Touhou.

Although it falls through because this hasn't been explained yet, the joke is that Sakuya, who is worried about her smallish chest just realized that the kindergardener who lives with her has breasts roughly the same size as her own, and is freaking out about it.
