It comes from 2ch, where a long wall-of-text rant was often replied-to with "お疲れ様", to the point that the mods banned the usage of "お疲れ様"; in response, 2channelers shortened it to 'otsu' and assigned a completely unrelated (but phonetically similar) kanji, 乙, to the phrase.

I'd assume on Nico it might actually be used legitimately, but considering its origins, 乙 probably means "Wow, you've got a lot of time on your hands" there.
If you see it actually spelled out with the original kanji or phonetically, it's probably meant genuinely - after all, if they're being sarcastic, all they have to type is 乙.
