Of course it hurts. You'd never see someone with pierced nips asked their partner to yank it like beer tab, won't you? There's a great risk you'll tear the nipple.

There’s a certain threshold where pain eventually turn into pleasure. Some people have a bit more tolerance than the other so there’s different levels for different people. Something of a defense mechanism for the human body.

This, like so many sexual things, is highly dependent upon the individual and the situation.
Any woman who isn't directly associated with the sex industry and keeps her pierced nipples probably gains at least some level of enjoyment from them being played with. The magnitude of that "play" is highly variable.
Some women don't even like having their breasts (let alone their nipples) touched at all at some points in their cycle (swollen and sore). But if they get aroused they may enjoy things very much because of extra sensitivity.
