Well, given the way the comic has progressed, it might not have been "all along." But that would probably make Mikuru/Mitsuuru feel even worse.

Actually, Haruhi thinks that Itsuki and Mikuri were cross-dressing all along - and thus, it is so. Both of them remember that they were originally the other gender, but the world functions as though they were not.

...Well, that would actually explain quite a bit. But... wow. Sucks to be Mikuru.

Poor Mikuru. Imagine if she had been more prone to take pride in her assets, she might as well just die of cerebral hemorrhage.
