Look at the fourth panel... at what Meiko is doing. It's like this:

3rd panel: Meiko jumps Kaito and starts beating him senselessly. Miku freaks/calls out to stop.

4th panel: Miku explains CM is a younger sister, not daughter. Meiko looks and realizes it. Kaito is blacking out.
