As I recall it, the "LOL" part of it comes from "Sweets (LOL)", a derisive term directed at gullible ladies who follow any trend they see in fashion magazines, primarily ones that say they can eat sweets and still lose weight.
Japanese Touhou fans simply combined those two elements to give Sanae a new nickname.
