Not only is it canon that Remilia is indeed childish, but, depending on how being a vampire works in the Touhouverse, there may even be a biological explanation for it.
Your brain really doesn't finish developing until you are at least 10 years old. You can of course learn and retain information well before then (even complex stuff), but the part of your brain that truly understands philosophical things, that handles all of the higher level stuff (worrying about the future, morality, self-actualization, etc.) doesn't finsih growing until about that age.
If Remilia was turned into a vampire rather than being bitten (or if she was born one and just ages very, very slowly and is the vampire equivalent of a small child in terms of how long she's lived), then its possible that the part of her brain that is responsible for maturity simply hasn't finished growing yet and may never finish growing.
She can still learn, retain information and experiences, but there is something truly mature adults possess mentally that she may never truly have. Its possible she and Flandre will forever be children. Of course its also just possible that being immortal and very powerful with nothing constructive to pass the deacdes by makes you very whimsical!
