It's a variation on the scene from book 2 when he nearly bitch slaps Haruhi while they're filming the movie.

Now it all makes sense. Quite frankly, I don't blame Kyon. I wanted to give her more than a slap when she said Mikuru was just a toy to her.

Fortunately, she becomes a lot more likable as the books progress.

Funny thing is, when a small kid gets hurt, they tend to be exactly like that - seconds of instense concentration as if they could not quite process what just happened before they start wailing.
