Speaking from personal experience, it's very VERY hard to urinate during or after orgasm in females. I suppose it's like a man not being able to pee with an erection. There is an overwhelming sense of the "need to bear down"? I guess? But that's what contracts the Skene's gland and pushes out the fluid.
