I think it's just the final chapter of their lives. Hopefully it's not the last comic. I want to see Reimu eat more weird stuff.

I like to think that they have been drinking all this time without a problem until Eirin says something about it and then reality hits them. Like the old joke of the self insert artist pointing out the laws of physics to a character and what they are doing is not really possible which anchors them to real life.

I don't think its liver failure per se, rather, I think Suika has a terminal illness, like liver cancer.

Let's hope she gets a liver transplant, provided they find a donor.
