Rather like a user on the GameFAQs UK forum. Every time a new user comes in and introduces themselves, regulars accuse them of being Izual, a guy who made so many alt accounts, it was once assumed that everyone on GameFAQs was Izual. When they deny it, or say they don't know who Izual is, they say "A typical Izual response", and it all sort of breaks down from there.
But yeah, that's roughly what's happened here. He's been clever in his own way for a while now, making puns in visual form, that now people will search for meaning in each and every one of his pictures.

Which, knowing Ao, would be the joke in the first place. Draw something, make everyone think there's something to be found, make them all waste their time trying to figure out the punchline...
Actually, thinking back on it, Ao Usagi probably would have just put Cirno's dress and/or wings on an apple if he was going for that kind of pun. This is probably just a reverse-mindfuck like Aleander said.
To this day, I think there's something wrong in this picture, but all I could think of is Cirno's reflection. I don't think she should have a reflection on the apple from that position...
