Kotomi-chan is always like that. It's the result of some childhood trauma. She's actually really cute/funny in the anime.

I think you're trying to figure out what was wrong with the head (too small). The eyes are pretty normal for kotomi.

I don't remember Kotomi's eyes ever being like that. Maybe during her giant freak-out over the car accident...I think it's the fact that the iris is so massive that makes them look more empty than usual here.

I think her head looks small because of the MASSIVE amount of hair on her head. I mean seriously, her head should fall off because of the weight.
