As someone who just had one randomly yesterday, let me assure you that for some people, they just happen. Superficial blood vessels, excessive sinus pressure, and dry air can cause them to happen at any time.
Maybe she just got in a fight, is in a bad mood and her nose won't stop bleeding... People keep staring, so instead of staring back she decides to stare at the window. The poor girl just wants to get home.
On a note, I had a friend that'd get nosebleeds very randomly too, BAD ones, specially if she kept her head down for too long.
Anyhow, I like this :3 reminds me of Touch Detective (for the NDS) for some reason... must be the colors.
Also, sometimes the change in air pressure at higher altitudes can cause nosebleeds. This is why the upper seats at sports stadiums are called "the nosebleed section".
I think I can make out lights from a tall building in the background, outside the train, so that might well be the case here.
